Here is a blank for anyone who wants to do their own. I usually reduce it 65% when I have finished making it.
I used for these Paint Shop Pro (PSP7) which is an old version. Any version would do the same.

PSP 7 is half way down the page.
Make sure you download the following otherwise your PSP7 won't work.
You will need the following two DLL files in order for some filters to work.
1. Plugin.dll .......... Unzip this file to C:/Windows folder
2. Msvcrt10.dll ...... Unzip this file to C:/Windows folder/System
It could be also be done in Photoshop.
There are places on line where you can add text to a graphic, the link I have is closed at the moment
